The program is designed to create a link between Mount Sinai Hospital Montreal and referring institutions.
The Palliative Care Unit at Mount Sinai Hospital Montreal seeks to provide a compassionate, dignified and comprehensive interdisciplinary approach to care of individuals at the end of life. The Unit addresses the diverse physical, psychological, social and spiritual needs that accompany this phase of life.
The Palliative Care Unit admits patients who are in the end stages of terminal illness, with a prognosis of less than three months. Patients seeking admission to Mount Sinai Hospital Montreal must have a signed referral from a physician and are usually referred by a CLSC, VON or hospital. Patients are admitted based on a priority list which takes each patient’s needs into account.
The Pre-Admission Program is a means of prioritizing patient admissions with a nurse assessing applications for Palliative Care. A nurse visits each patient prior to admission and then maintains regular contact to ensure they are being well managed. As a result, when admission becomes necessary, it can be arranged in a timely fashion.
