Home Care Program
Mount Sinai Hospital Montreal’s Home Care Palliative program was created in 2001 with the goal to offer patients the opportunity to remain at home with dignity, while receiving adequate care. Patients have the opportunity for admission in the Palliative Care unit should their home situation become too difficult to manage.

Patients are referred to Mount Sinai’s Home Care program by various hospitals in metropolitan Montreal. To be eligible to participate in the program, patients must reside in the CLSC territories of Notre-Dame-de-Grâce, Côte-des-Neiges or Metro areas.

Mount Sinai Hospital Montreal offers medical and nursing care and psychological support in addition to the services offered by the CLSC, which has the mandate to offer home care to the population in its territory.

The main goals of Mount Sinai’s Home Care Program include:
  • Allow patients to stay at home as long as possible with good symptom management
  • Keep the patient and family well-informed about disease progression in order to be able to make more informed decisions
  • Coordinate care with families
  • Develop Palliative expertise with other home care professionals
  • Coordinate care with CLSC nurses
  • Cooperate with hospitals and the CLSC to offer quality care
  • Educate health care workers
  • Develop knowledge of Palliative Care with group education sessions
  • Support first line doctors and nurses
  • Offer nursing internships for CEGEP and university students
  • Participate in university research for the development of Palliative Care
