The interdisciplinary team in the Long-Term Care unit aims to create a comfortable, therapeutic and warm surrounding while meeting the clinical needs of each patient. Family members can rest assured that their loved ones are in a safe, stimulating environment and receive regular specialized medical follow-up. Psychologists help residents and their families adapt to and cope with the transition to living in a Long-Term Care unit. At the same time, involvement of the Recreology department is strong as patient stimulation and socialization are extremely important.
At any given time, 57 residents live in the unit. Within the Long-Term Care unit, a team of nurses is always on the premises to ensure that patients receive the best quality care. Patients are encouraged to consult a range of professionals at the Hospital based on their individual needs.
Patients seeking admission to the Long-Term Care unit are generally dependent individuals who have lost their autonomy. Anyone in this situation, over the age of 18 may apply. Prospective patients are requested to consult a social worker or their nearest CLSC to have their name placed on the regional waiting list of the Agence de Santé et de Services Sociaux de Montréal. A placement committee of the Agence will then determine the most suitable institution for placement according to the care need. Patients with respiratory disorders and/or palliative care needs are prioritized in the admissions process for Mount Sinai.
Once the patient is deemed eligible for placement at Mount Sinai, a nurse from the Hospital will visit them to validate their candidacy for the Long-Term Care unit. Upon approval of admittance the patient and their family are given a proper orientation of the facility.
