Admission to any unit within Mount Sinai Hospital Montreal requires referral by a physician.

Upon admission to Mount Sinai, you will be contacted via telephone, to confirm a specific date for admission. On this day, you must arrive to the Hospital at the designated time with the following documents:
- Medicare Card
- Mount Sinai Hospital card (if necessary)
- Proof of insurance (if necessary)
All newly admitted patients are greeted at the Hospital with a welcome package. Members of the Auxiliary visit new patients within a week of their arrival and present them with this special gift of various toiletry items.
Below is a guide to Mount Sinai Hospital Montreal for new patients: Valuables The Hospital cannot be held responsible for any loss or breakage of valuables, such as jewelry or money. It is advised that patients keep only a small amount of cash at their bedside. Valuables can also be locked in safes available in the Medical Records department. Safes can be accessed from 10AM to 12PM and 3PM to 5PM.
In order to avoid the loss or damage of glasses and other prosthetics, it is recommended that they be stored carefully when not in use. If these prosthetics are not essential during your stay, it is suggested that you leave them at home.
Temporary Leaves Patients are not permitted to leave the Hospital without the authorization of a physician. If you are granted a temporary leave, you must sign a special form prior to leaving the facility. Your bed will be temporarily reserved in your absence. If you are staying in a private or semi-private room, you must continue to assume financial responsibility during your leave, even if you have been temporarily transferred to another hospital.
Care & Treatment Consent By law, Mount Sinai Hospital must receive your consent prior to performing medical treatment. Before signing this document and making a decision, ensure that you have been provided all of the necessary information from your physician.
Cost of private and semi-private rooms For short term stays, patients requesting a private or semi-private room must pay a supplemental fee. If you have a personal insurance plan, this supplement can be claimed, in whole or in part, by your insurance company. Should this be the case, you must present your insurance card upon admission. It is your responsibility to assume proper medical coverage. If you do not have personal health insurance, you must pay these supplemental fees five days prior to admission. You may do so at the Accounting department in room B113.
If you are covered by a partial personal health insurance plan, you must pay the difference of the expected cost. If necessary, Mount Sinai Hospital will reimburse you for the excess sum by cheque as soon as possible.

Departure Hours On the day of your discharge, you are requested to leave your room as early as possible so that another patient awaiting admission can enter Mount Sinai Hospital. Please visit the Accounting department to settle the final administrative fees.
Medication Without exception, Mount Sinai will provide all medications needed throughout your stay. However, it is requested that you bring all medications from home for identification and prescription by a physician at the Hospital. It is possible that the physician will modify your medications. If this is the case, you will be given new prescriptions which must be filled, at a pharmacy, prior to your departure from the Hospital.
Presence in your room You must advise the nursing station and inform yourself of missed examinations when you are absent.
Fire In case of fire, stay calm and wait for instructions from Hospital staff, specially trained in responding to this type of situation.
Call Bell A call bell is installed near beds and in the washrooms. If in need, do not hesitate to ring the call bell.
Parking Parking is available in the lot adjacent to the Hospital for $6/day.
Outpatient Clinics It is possible that a physician will refer you to an outside clinic. In this case, you must arrange an appointment in advance. You may contact the Ambulatory department at (514) 369-2222 extension 1241.
Taxi In the main entrance of the Hospital you will find a telephone directly connected to a central taxi service.
Telephones The cost of in-room telephones is included for short term stays in semi-private and private rooms and is available for $2.15/day in 4 bed rooms. The cost of an in-room telephone for a long term stay is $37.03/month in all rooms.
To make a long distance call, contact the operator by dialing 0, you will be billed for the incurred charges.
Television In-room televisions are available at a cost of $4.07/day in short term care and $64.84/month in long term care.
Long term residents may provide their own televisions and pay $22.88/month for cable.
Smoking Smoking is prohibited in all areas of the Hospital. Individuals smoking on the premises will be considered to have committed an offense.
Visitors To ensure privacy and quiet, patients sharing a room with others are limited to two visitors at a time. Hospital visiting hours are from 8:30 AM – 11:30 PM daily. Visitors can stay overnight in the Palliative Care unit. Children are welcome but must be accompanied by an adult.
Medical Records Patients and their legal representatives may consult their medical records. Requests must be made in writing, detailing the need of these documents, and should be submitted to the Medical Records department.
Death Certificates Certain legally authorized individuals can request death certificates with the appropriate written proof. Requests should be made to the Medical Records department. Death certificates are free however a copy of an act of death costs $25.
